Ride/Rider Requirements

All riders must provide a copy of a valid drivers license, valid insurance information, a signed release form, and the emergency contact information along w/a list of all medications and physician’s contact information.

Below are the two required forms that each rider must have submitted to John McKendree prior to joining with the Flame of Freedom® group.  For your convenience you can download the form in either a .pdf (in order to open the .pdf you must have Adobe Reader or equivalent .pdf reader) or a .doc (MS Word) file.
